Choose this if you got the app from a physiotherapist or clinic. If there is only one physiotherapy option nearby, we’ll take you straight to the next page
Pharmacy/Care Home/Occupational Health
In some areas, you might see pharmacies, care homes or libraries. Pick the place where you got the app; you won’t need to enter extra details
None of the Above
If none of these fit, choose ‘None of the above’. We’ll ask you to register under your GP practice or pick ‘I do not have a practice
