Can't find your practice in the list?
Type the name of your GP practice exactly as it appears on your practice’s website. If your practice is part of a group, we may only have one entry for the group. If you can't find your exact practice location but can find your group, then continue with the group option.
Did you select the right area?
If you had a choice between multiple regions, try the other region options if you can’t see your practice.
